Eligibility: The prize draw is open to individuals who have completed a feedback form at All Health Matters clinic, or who have left feedback on Google or Facebook regarding All Health Matters. Employees of All Health Matters and their immediate family members are not eligible to participate. Participants must be residents of the United Kingdom and aged 18 years or over. 

Entry Period: Feedback must be submitted within the three months prior to the month of the prize draw. The prize draws will take place in January, April, July, and October each year. 

How to Enter: No purchase is necessary to enter or win. A purchase does not increase the chances of winning. Participants will automatically be entered into the prize draw upon leaving feedback through any of the specified channels. 

Prize: Each winner will receive their choice of a £50 Amazon.co.uk or Boots gift card. The prize is as stated and no cash or other alternatives will be offered. The prizes are not transferable. The draw is in no way related to Amazon, Boots, Google, or Facebook. 

Selection and Notification of Winner: Winners will be chosen at random using a random number generator from all eligible entries received. The draw will be conducted by a representative of All Health Matters within 30 days following the end of each quarter. The winner will be notified by email within 7 days of the draw. If the winner cannot be contacted or does not claim the prize within 14 days of notification, we reserve the right to withdraw the prize from the winner and pick a replacement winner. 

Opting Out: If you wish to submit feedback but do not wish to be entered into the prize draw, please make this known either via the feedback channel or by email to admin@allhealthmatters.co.uk.  

Claiming the Prize: Winners will be sent an email to the address provided with instructions on how to claim the prize. If a winner does not respond within 14 days of notification, a new winner will be selected in accordance with these terms and conditions. 

Data Protection and Publicity: By entering this prize draw, participants agree to the use of their names and images in any publicity material. Any personal data relating to participants will be used solely in accordance with current UK data protection legislation and will not be disclosed to a third party without the individual’s prior consent. 

General: All Health Matters reserves the right to cancel or amend the prize draw and these terms and conditions without notice in the event of a catastrophe, war, civil or military disturbance, act of God or any actual or anticipated breach of any applicable law or regulation or any other event outside of the promoter’s control. Any changes to the prize draw will be notified to entrants as soon as possible by the promoter. 

Governing Law: These terms and conditions shall be governed by English law and the parties submit to the exclusive jurisdiction of the courts of England and Wales.
